Young people
Investigation into ways that State government departments and authorities can prevent or reduce suicide by young people
Report. The objectives of the investigation were to analyse, in detail, deaths of young people who died by suicide notified to the Ombudsman WA office, comprehensively consider the results of this analysis in conjunction with the relevant research and practice literature, undertake consultation with government and non-government stakeholders and, if required, recommend ways that agencies can prevent or reduce suicide by young people.

Investigation into an incident of alleged excessive force used by authorised officers
Ombudsman report into investigation of alleged heavy-handed conduct of authorised officers on the public transport network. The investigation concluded that the force used by an authorised officer on a teenage girl suspected of evading a $2.50 fare, was excessive and amounted to degrading treatment and a breach of the Victorian Human Rights Charter. It also concluded that the department's internal investigative processes, including a review carried out following a public petition, continued to assert, without interviewing any independent witnesses, that the officers involved had complied with training methods and done nothing wrong.
